Objectives: This study aimed to investigate the effects of transcranial direct current stimulation (tDCs) on resting-state electroencephalography (EEG) mean frequency and motor recovery in patients with chronic stroke.
Methods: Thirty individuals with chronic stroke were randomly allocated to receive either real or sham anodal tDCs combined with task-oriented training (TOT). The real-tDCs group received 2 mA anodal stimulation over the motor cortex for 20 minutes per session, five times per week, for three weeks. EEG mean frequency and the Fugl-Meyer Assessment (FMA) of the upper extremity were used as the main and secondary outcomes, respectively.
Results: Both groups demonstrated a gradual reduction in EEG mean frequency over time, with a more pronounced decrease in the sham group. This difference was statistically significant only at follow-up (P = 0.02). The FMA scores improved significantly in both groups (P < 0.001), with a greater improvement trend in the real-tDCs group; however, intergroup differences were not significant.
Conclusion: Although the association between EEG mean frequency and motor performance remains inconclusive, these findings suggest that anodal tDCs may modulate cortical activity and support motor recovery in chronic stroke patients.
